    NOTE: SUBMITTING THIS FORM DOES **NOT** GUARANTEE YOU ARE BEING ROSTERED TO A PARTICULAR TEAM, NOR THAT YOU WILL BE A TEAM OFFICIAL ON ANY TEAM. All applications are being reviewed. We are asking for Expression of Interest ONLY. Applications that do not submit VALID CRC, Respect in Sports for Activity Leaders and CATT will not be considered until certs are up to date.

  • IMPORTANT: If you do not have a Hockey Canada ID number, a CURRENT/VALID CRC, RIS-AL, and CATT, you will not be able to complete this form. Please return when they have been completed. 

    What does "current" mean? 

    • It means that the expiry date is NOT in the year 2025 or prior.  Submitting certs with expired dates will just cause back and forth. Please do not complete this until all required items are current.  APPLICANTS WITH EXPIRED CERTS WILL NOT BE CONSIDERED UNTIL UP TO DATE.

    I don't see Head/Assistant Coach/On-Ice Volunteer in the Role cells below.

    Can I use my child's hockey ID number? 

    • No, you cannot. 

    I don't know my Hockey ID number; how can I find it?

    • You will need to look up your hockey ID number by setting up your Spordle account.  LINK TO CERTIFICATIONS PAGE
    • IF YOU HAVE NEVER PARTICIPATED IN HOCKEY, EVEN AS A PLAYER, EVEN IF IT WAS WHEN YOU WERE A CHILD, PLEASE REACH OUT TO YOUR DIVISION MANAGER.
