Dance Class Enrollment and Release Form
You are participating in dance, movement, or other activities or classes (the “Activities”) offered by The Denver Diamond Dolls LLC (“Diamond Dolls”). You understand that during these Activities, Diamond Dolls may use discretion to provide you with instruction and direction to support your dance, physical movement, or other creative practices.
Diamond Dolls offers Activities primarily to participants who wish to celebrate their feminine side and are 18 years of age or older. Participants aged 16-17 may be permitted to participate only with the advance approval of Diamond Dolls. Please email Denverdiamonddolls@gmail.com for approval if you have not yet done so.
You agree that your participation in the Activities suggested by Diamond Dolls is at all times voluntary and that you are solely responsible for your own health and well-being.
Acknowledgment of Risk
I understand that all physical movement, including dancing, involves inherent risks. By participating in the Activities, you may suffer injuries resulting from tripping and falling, overstretching, misuse of a prop, strenuous physical exertion, or moving your body beyond your physical limitations. you acknowledge that your participation may also cause or aggravate a physical injury or condition or otherwise result in more serious injuries, including but not limited to high blood pressure, cardiovascular incidents such as heart attack or stroke, fainting, broken bones, and injury to sensitive areas such as the back and neck.
Preparing a Safe Space for Virtual Sessions
If this session is virtual, you are responsible for assessing your surroundings before dancing. Diamond Dolls recommends that participants ensure small children and pets are not in your dancing space and that furniture, items, and other trip and fall hazards are addressed before any virtual session begins.
Assumption of Risk & Release of Liability
I understand that the Activities involve some risks and agree that Diamond Dolls, its owners, contractors, employees, volunteers, and representatives and Denver Dance Center and/or any other venue where the Diamond Dolls practice or perform and the owners, contractors, employees, volunteers, and representatives of such venues (collectively, “the Diamond Dolls Parties”), are not responsible in any way for any injury you may suffer in or as a result of your participation in the Activities, whether these injuries result from the negligence of Diamond Dolls Parties or from some other cause. You also acknowledge that any loss or damage of personal property is not the responsibility of The Denver Diamond Dolls. You agree to hold Diamond Dolls Parties harmless against claims related to such injuries.
Photo/Video Release
I understand that Diamond Dolls from time to time may photograph, video, or otherwise record the Activities for marketing purposes or for selling pre-recorded classes in which you were a participant. These images may include you or (if you are participating by video with other people in the frame) minor children for whom you are legally responsible. You consent to such photography or recording and you grant Diamond Dolls the right to use your likeness for any purpose, including advertising and commerce, without restriction and without compensation. You waive any right to approve the images, their use, or any text that may appear in connection with the images.
Refund Policy and Missed Rehearsals/Performances
Diamond Dolls programs are typically group classes with limited enrollment. You understand that after you enroll in a program, Diamond Dolls will turn away other participants based on your commitment and thus your enrollment fee is nonrefundable. Any sessions (including performances) that Diamond Dolls provides under this Agreement but that you fail to or am unable to attend will be forfeited. You are not entitled to a refund or a rescheduled performance if you are unable to attend the performance as scheduled by Diamond Dolls.
Diamond Dolls Guidelines and Expectations
1. Please look to the Board members/teachers for direction and guidance during class, performances, and any event attended by The Diamond Dolls. If you have feedback or suggestions for the group, please communicate with a member of the Diamond Dolls board. The Diamond Dolls strives to be an inclusive group that is constantly improving - your input is important. You may also email us at denverdiamonddolls@gmail.com. 2. Our class time is dedicated to learning and we respect our teachers’ time and energy in leading the rehearsal. Please raise your hand with questions and keep side conversations outside of class time. If this session or event is virtual, please keep mics on mute unless you have a question or are having an issue. You can also type any questions or comments in the chat.
3. Take ownership of your education; practice outside of class. Ask for help when you need it.
4. Performing is always optional! You are welcome to attend rehearsals without participating in the performance but please let your instructor know as soon as possible if you have decided not to participate in the performance. Participation in the performance is reserved for participants who have thoroughly prepared the routine and Diamond Dolls reserves the right to restrict performers from participation if they have not regularly attended rehearsals and thoroughly prepared for the performance. If you regularly miss rehearsals and/or performances without clear communication with your instructor, Diamond Dolls may prohibit you from participating in future sessions.
5. Diamond Dolls may provide a costume for your performance, or may require you to assemble your own costume. If Diamond Dolls provides a costume, you agree to wear the costumes the way they are intended to be worn and treat them with care. Some costume elements are irreplaceable. Make sure to return all items immediately after a performance. Check with the performance manager while at the event for return instructions. You are responsible for the cost of any costume in your possession that is damaged, lost, or stolen.
6. Diamond Dolls owns all Diamond Dolls choreographies. You do not have permission to teach or share choreography practice videos with anyone other than the other participants in your session. Diamond Dolls may make past routines available to interested students for an additional fee.
8. You agree to notify Diamond Dolls immediately if you are experiencing cold or flu-like symptoms, or if public health standards direct the Student’s self-isolation due to testing positive for COVID-19, experiencing COVID-19 symptoms, or exposure or probable exposure to COVID-19 before an in-person session, rehearsal, or performance. Diamond Dolls retains the right to end in-person Services immediately if a participant is exhibiting cold or flu-like symptoms, or if Diamond Dolls learns that a participant has recently tested positive for COVID-19, is presumed positive or awaiting test results for COVID-19, or is displaying symptoms of COVID-19. In the event the Diamond Dolls end a session early under the terms of this clause, the participant will not be entitled to a refund, and the participant will hold the Diamond Dolls harmless for incomplete services.
