WCAB Notice to Cancel Membership

Submit this form to cancel your swimmer's WCAB Membership.  This form must be submitted no later than 60 days prior to the termination date.

Submit a separate Notice to Cancel Membership for each additional swimmer associated with your account.

All forms received within 60 days of the cancellation date will be effective only after the subsequent month (for example, if you submit the form on the January 15th asking your swimmer to be placed on LOA from March 1, the LOA will be effective only from April 1 and you will be charged the regular monthly fee for March).

All cancellations will be processed on the last 2-3 days of the month. Until this form has been processed, the TeamUnify invoice notification WILL NOT reflect the termination.

Cancellation Policy

By submitting this form, you are cancelling your swimmer's WCAB team membership.

• Notice to Cancel Membership requires a 60-day notification to the team.
• All forms received within 60 days of the cancellation date will be effective only after the subsequent month (for example, if you submit the form on the January 5th asking your swimmer to be placed on LOA from March 1, the LOA will be effective only from April 1 and you will be charged the regular monthly fee for March).
• There is no prorating of dues for missed days of practice, family vacations, or any other reason.
• Volunteer commitments and dues are not waived during the 60-day notification period.
• Member families who have a swimmer leave the team to join another USA-S team will be required to forfeit all of their family's memberships.  This is to protect the team spirit of our existing members.  This policy does not apply to families joining the team (for the first time) who may have children on another team.
