CLEARANCE & BACKGROUND CHECK (Required)

Due to the nature of our work serving children and families with disabilities, ALL volunteers 18 and older are required to complete a Pennsylvania background check and child abuse clearance before participating in volunteer activities.

There is no cost to the volunteer to acquire these forms. A member of our board will provide the necessary links and instructions. Completed clearance documentation must be submitted to: thenestproject.us@gmail.com before volunteer activities can begin.

SIGNATURE & LIABILITY AGREEMENT

I understand that by submitting this form, I am expressing interest in volunteering with The NEST Project and this will be a volunteer basis position and may be contacted for onboarding and orientation.

I acknowledge that upon approval as a volunteer, I am volunteering at my own discretion and risk. I agree that The NEST Project, its board members, officers, and affiliates shall not be held responsible or legally liable for any injuries, accidents, damages, losses, or claims that may arise from any events I participate in or any donations I deliver on behalf of the organization.I assume full responsibility for my actions and participation.

I understand that I am not covered by any insurance or workers' compensation through The NEST Project.

I also understand that if I express or engage in any behavior deemed threatening, discriminatory, offensive, or inappropriate toward the Board members, fellow volunteers, or any individuals served by the organization, or if I act in a manner that goes against the mission and values of The NEST Project, the Board reserves the right to terminate my volunteer involvement immediately without further notice.

Due to the sensitive nature of the individuals and families we serve, I agree to: Maintain complete confidentiality regarding any information I see, hear, or handle while volunteering, including names, addresses, medical information, disabilities, and personal stories (unless family or agency signs off with permission Not disclose, copy, discuss, or share any identifying information about clients, donors, or organizational matters unless explicitly authorized by a member of the Board. Understand that any violation of this agreement may result in immediate termination of my volunteer role.
