  • SECTION I: Benefits Eligibility

    Note: If you are on a multiple-point charge, please fill out Section I based on your combined appointment status.
  • If a waiver document is not on file, contact Lorena Ossa at lossa@trinitygmc.org to recieve a waiver and instructions. 

     

  • SECTION II: Cash Salary & Allowances

    Note: If you are on a multi-point charge, please fill out Section II based only on the church selected above. This is your cash salary and any allowances that do not require receipts. Include housing allowance only in Section III below.
  • SECTION III: HOUSING

    Note: The Housing Allowance amount is set by the church in addition to the salary included in Section II above. It can be paid out with the regular paycheck or in a separate check. Lay ministers are not eligible for Housing Allowances.
  • Note: If you are on a multi-point charge, please fill out Section III based only on the church selected above.

  • SECTION V: LOCAL CHURCH EXPENSES FOR CLERGY/MINISTERS

    These are additional expenses (if you are eligible) the local church will cover on your behalf but are not income to you. Enter 0 if you are not eligible or have waived eligibility.
