Requirements for self-administration of medication
Brainbox Science staff are not trained in medication administration and cannot take responsibility for administering medication to students. Campers who need to take medication during the day or have medication available in case needed must be developmentally capable of storing their medication, knowing when to take their medication, and self-administering that medication.
If there would be health consequences if medication were delayed, missed, lost, or taken incorrectly, please consider whether Brainbox Science would be a good fit. We want your camper to be healthy and safe!
Students who are developmentally capable, will be allowed to self-administer medication, subject to the following:
- This Self-Medication Agreement form must be submitted for all self-medication and must be approved by a Brainbox Science administrator.
- All medication must be kept in its appropriately labeled, original container. Prescription labels must specify the name of the student, name of the medication, dosage, route, frequency or time of administration, and any other special instructions.
- Sharing and/or borrowing of medication is strictly prohibited and may lead to removal from the program.
- Permission to self-medicate may be revoked if the student violates these regulations or shows evidence that they are not developmentally or behaviorally capable of self-administering medication.
By completing this form, you are indicating that your student can safely carry the medication in their backpack, open the medication container, and remember when and how to take the medication. Brainbox Science staff will not be able to carry or store medication, remind your student to take medication, or help with self-administration. If possible, we recommend sending your camper with only the medication they will need for each day.
