RELEASES & CONSENT
This document is to be signed by participants who have reached the legal age of majority (age 18).
LIABILITY RELEASE
I hereby release and discharge the Church of the Nazarene, Inc. and its affiliates, along with any other chaperoning adult employees or volunteers of Nazarene Missions Teams, its agents, employees, officers, directors, affiliates, successors, assigns and all others from any and all claims, demands, expenses, personal injury, wrongful death, causes of action, lawsuits, damages, and liabilities of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to participation in any activities associated in any way with Nazarene Missions Teams and it’s projects. I have full knowledge as to such activities and I have full knowledge of the possible risks involved.
MEDIA CONSENT FOR ADULTS
I grant to The Church of the Nazarene, Inc., a Missouri nonprofit corporation, and its employees, officers, agents and assigns (hereinafter referred to as “Nazarene Church”), the right to use, reproduce, adopt, distribute, and publish photographs, video, audio, related content or personal information, including but not limited to my first and last name and general area of residence (“Content”) taken by Nazarene Church at or during a Nazarene Missions Teams project or that I have taken independently and voluntarily submitted to Nazarene Church for use by any of the ministries of the Nazarene Church for purposes of informing and promoting the work of the Nazarene Church and its ministries. I understand and agree that the Content(s) may be used in whole or in part, at any time, in composite or distorted form, in color or otherwise and may be published through any medium including, but not limited to, printed medium, video and/or on the internet. I waive any right to inspect or approve any finished product, advertising or other copy that uses the Content. I agree to indemnify Nazarene Church from and against any claims, losses, judgments, settlements, expenses and reasonable legal fees, incurred by Nazarene Church by reason of any claim of invasion of privacy or trespass created in the taking or publication of the Content(s). I also agree to indemnify Nazarene Church for any such amounts of arising from my breach of any covenant, representation, or warranty of this agreement.
REGISTRATION & VERIFICATION TERMS AND CONDITIONS
By registering for or participating in Nazarene Missions Teams projects, you confirm that you have read, understand and agree to these Terms. The Terms may be modified at any time by posting a revised version on the Nazarene Missions Teams website or by providing other notice to you. By participating in the project, you agree to the latest version of these Terms.
1. Verification. To complete the team registration for the project, you (as a team participant) must complete the participant verification, prior to any deadlines described in the Nazarene Missions Teams project participant email for the team registration.
2. Trip Cancellation. Berkley will reimburse the Covered Person for the unused, non-refundable, prepaid Trip payments/deposits paid by the Covered Person for his or her Trip up to the Benefit Maximum shown above, if the Covered Person is prevented from leaving on his or her Trip due to a Covered Reason. The Trip cancellation date must be a date prior to the Trip departure date. (See Berkley Insurance Guidelines & Information sheet for complete list of coverage reasons for cancellation).
3. Code of Conduct. You agree to behave in such a way as to create a safe, supportive learning environment while traveling to and from the project, at the project site and any related events during the trip. You agree not to engage in disruptive speech or behavior or otherwise interfere with the project objective or other individuals’ participation in the project. You will comply with all applicable laws and agree not to engage in any form of harassing, offensive, discriminatory or threatening speech or behavior, including relating to race, gender, sexual orientation, religion or disability. Nazarene Missions Teams may require participants engaging or threatening to engage in such behavior to leave the project site or related event immediately and such participants will not be eligible to receive a refund of any money paid to Nazarene Missions Teams related to the project. If you witness such behavior, please promptly contact a Nazarene Missions Teams staff member so direct action can be taken.
AUTHORIZATION FOR CONSENT
I understand that the terms herein are contractual and not a mere recital, that this instrument is legally binding, and that I have voluntarily signed this document. I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS CONSENT, WAIVER AND RELEASE BY READING IT BEFORE SIGNING IT ON BEHALF OF MYSELF AND MY HEIRS.
I have reached the legal age of majority and am signing on my own behalf.
