Fly DBQ $100 Rebate Request Form
Thank you for choosing Dubuque Regional Airport! Thanks to Dubuque Initiatives, book a round-trip ticket between Dubuque (DBQ) and Chicago (ORD) on Denver Air Connection and receive a $100 rebate. One-way tickets are also eligible for a $100 rebate, but the maximum rebate is $100 per trip per traveler (two one-way tickets making a round trip do not qualify for $200).

You may claim rebates on multiple round-trip (or one-way) bookings made between now and September 30, 2025. Travel may occur after September 30 as long as tickets are purchased by that date.

Please complete the form below to request your rebate:
- Provide your contact information so we can mail your rebate check.
- Upload proof of purchase for each booking you are claiming a rebate on. Multiple uploads (up to 5) are allowed.
- Once submitted, your request will be reviewed and processed by Greater Dubuque Development Corporation.
