Confirmation 2025-2026 | St. Alphonsus Rodriguez Church
The primary responsibility of the sponsor is to provide the candidate with support and guidance in his or her faith journey and to “take care that the confirmed person behaves as a true witness of Christ and faithfully fulfills the obligations inherent in this sacrament” (Canon 892).

All sponsors:
- Must be at least 16-years-old
- Must have received all of the Sacraments of Initiation: Baptism, Confirmation, and First Communion
- Cannot be a parent
It is the responsibilty of the sponsor to complete this form! Parents should not be completing it on the sponsor's behalf.
