Laced Luxury Monthly Lash Membership Agreement & Consent Form

 Membership Details

• Membership Fee: $130/month
• Membership Includes: 1 Full Set OR 2 Lash Fills per month
• Memberships are non-transferable and valid only for the client listed above. 
• Services must be booked in advance and are subject to availability.

 

 Terms & Conditions

Monthly Billing:

The membership fee will be billed automatically each month on the date of enrollment to the card on file. 

If the card on file does not process, you will be contacted and must provide a valid card in order to process your monthly payment. Failure to do so will result in failure to schedule future appointments and automatic cancellation of future scheduled appointments. 

Cancellations/Pauses:

Membership may be canceled at any time with a 14-day written notice prior to the next billing cycle.
Memberships may be paused for up to 1 month per calendar year with prior notice.

Unused Services:

Any unused lash sets/fills do not roll over into the following month.

Late or Missed Appointments:

Cancellations or reschedules require 24-hour notice.
No-shows or late cancellations will result in the loss of that month’s included service.

Additional Services:

Any services beyond the monthly membership package will be charged at the regular service rate. 

 

Consent & Acknowledgment:

I understand that lash extension services carry some risk of irritation or allergic reaction.
I release Laced Luxury and its technicians from any liability related to my lash extension services.
By signing below, I agree to the terms of the Monthly Membership.
