This is to certify that I do hereby grant permission for me and/or my child to participate in all trips at Fairview Baptist Church from September 2025 through August 2026.
1. I further agree to release Fairview Baptist Church of Greer, South Carolina, (hereinafter “Fairview”) and each of the sponsors, individually and collectively, from all liability claims or any and all personal injury, physical and mental pain and suffering, mental disorders, property loss, or property damage which may occur to the above-named participant or participants’ property while engaged in said activities/trips.
2. I further authorize the sponsors to treat, discipline, restrain, and authorize reasonable and necessary medical care for the above-named participant, including but not limited to any emergency surgical procedure or hospitalizations if the same should become necessary where so ever the participant may be located.
3. I acknowledge the undersigned shall be liable and agree to pay all cost and expenses incurred in connection with any such medical and dental services rendered to the above participant pursuant to this authorization.
4. I understand that should it be necessary for the above minor to return home due to medical reasons or otherwise, the undersigned shall assume all transportation costs.
5. As the undersigned, I understand it is my responsibility to update the emergency information contained in this participant form as necessary.
6. I consent and give my permission for the church to use any photographs and/or video of the above participant for use in the church and/or any promotional material for the church.
The permission, release, and authorization granted herein are given for and in consideration of Fairview sponsoring this activity/trip/sport and permitting me and/or my child to participate. It is my expressed desire that the above-named person participate in this activity.
