If you do not have an email address, billing notices will be mailed to the mailing address above. The library cannot guarantee prompt delivery of billing notices mailed through the Post Office.

Any false statement or misrepresentation will result in immediate suspension of borrowing privileges and the library may take any appropriate legal action that is available to it.  I agree to follow the provisions of the Adult and Youth/Children's Circulation Policies and Procedures of Marion Public Library.  I agree to pay all fees charged to my card and to pay cost of collection.  I also agree to notify the library of any change in my name, address or phone.  I will present my card to check out materials.  I understand that my Library card is non-transferable, and I will not loan my card to anyone else.  I am responsible for any item borrowed with my card or against my card number; if my card is lost or stolen, I will report it immediately.

    Within 30 days of completing the online form, bring a photo ID and proof of address (if your photo ID has your current addresss, that's all you need) to the library to complete your application and receive your library card!

  • Permission For Spouse to Use Library Card
     
    I give my spouse         permission to use my Marion Public Library card. I understand that this is an individual card and the use of the card would be limited to the borrowing limits of the individual cardholder (For example: only 7 fictional videos would be permitted to be checked out at any given time. Each person could not check out 7 fictional videos.).
    I will notify the library if I no longer desire my spouse to use my Library card.

Applicants under the age of 18 must have this form signed by parent or other adult sponsor who accepts financial responsibility for all items borrowed against this assigned card number.
