  •  I understand that I am submitting an audition and I will be notified if I have been selected to participate in the Winter Production Class by finalizing this submission with payment of tuition. All information gathered in this form is to manage expectations and expedite the process after selections are made.

  • PRODUCTION CONTRACT

  • * I acknowledge that I am auditioning to participant in the 2025 Winter Production Class of Beetlejuice, Jr. only.

  • * I acknowledge that there is an IN-PERSON MANDATORY audition on October 2nd.

  • * I acknowledge that, if selected, tuition ($475, all inclusive) is due by end of day October 6th. Failure to pay tuition by October 6th will result in removal from the class. All invoices are issued through Square.

  • * I acknowledge that, if selected, there are NO CONFLICTS allowed for the audition (Oct. 2nd), cast meeting (Oct. 16th), or within two weeks of the performance (Dec. 18th thru Jan. 4th).

  • * I understand that, if selected, there is a parent meeting on Oct. 16th @ 6:30p. That is also the last day to order Add-on Show T-Shirts.

  • * I acknowledge that, if selected, the participant is only allowed 2 conflicts during the entirety of the Beetlejuice, Jr. process, due in writing by October 6th, or they will not be excused. Please email to candy@stagecenterla.com.

  • * Rehearsals are Thursdays from 5:00-7:00 and Saturdays from 2:00-5:00. My child will be present at all scheduled rehearsals except those for which I had approved conflicts, or in the event of serious illness or death in the family, for which I will provide appropriate documentation in a timely manner preceding the absence.

  • *We work on a 2-strike rule. Excessive tardiness for arrival or pickup, unexcused absence, and behavioral issues during rehearsals will result in a strike. 1st strike is a warning, 2nd strike is probation, and 3rd strike will result in dismissal from the production class. 

  • *If selected I will request to join the FB group, and understand this is the primary source for announcements and updates of critical information.

  • *We cannot issue refunds for any reason.

  • SoPA LIABILITY

  • I    *   *  , Parent/Guardian of (The "Participant")   *   *  , acknowledge my understanding that performing arts training is a potentially dangerous activity. I recognize risks are inherent in such training including but not limited to serious physical injury. I hereby agree that the Participant may engage in activities of Stage Center's School of Performing Arts. 

    I further agree to indemnify and hold harmless Stage Center, its instructors, officers, directors, agents, and employees (collectively Stage Center, Inc.) against liability resulting from any injury that may occur to the Participant while participating in activities of Stage Center. I also agree to indemnify Stage Center for any damages incurred bring in whole or in part from any claims, demand, action or cause of action by the Participant. 

    I authorize any representative of Stage Center to have the Participant treated in any medical emergency during his or her participation in activities of Stage Center. Further, I agree to pay all costs associated with medical care and transportation for the Participant. 

    I hereby give my consent to Stage Center, Inc. to use my child's image via submitted headshot or official photographs from productions to be used for marketing and promotional purposes.

    I understand that failure to pickup my child within 10 minutes of the conclusion of his/her class will result in a fee of $1.00 per minute past such 10 minute period, and I agree to pay said fee prior to the next class.
