Georges P. Vanier School - Parental Consent for Athletic Teams
Your child has been selected as a member of one of Georges P. Vanier School's athletic teams. As part of an athletic team representing the school, we have certain expectations of your child in terms of attitude, effort, and conduct while he/she is participating in their events and while they are out on road trips.
All athletes are expected to:
- Obey directives of coaches and supervisors at all times.
- Respect curfews set by supervisors and coaches.
- Observe school dress code while on trips.
- Not to leave designated areas without the permission of a coach or supervisor.
- Be respectful and obedient toward supervisors placed in charge at schools where we are attending events.
- Not bring or consume illegal drugs, alcohol, or bring contraband of any kind to events or trips.
- Show up for practices on time, with a positive attitude and work ethic.
- Respect decisions made bycoaches with regard to who plays in any given game or match or who participates in any given event.
- Abide by all applicable school rules while on their interschool activity because all athletic team members are considered to be "in school" during the entire course of the activity, including times when they are not playing.
As a member of a school athletic team it is understood that your child will often be going on road trips. When you sign this form you will be authorizing us to take your child on all trips pertaining to the athletic team to which they are a member. If you do not wish your child to participate or travel to a given event, please contact your child's coach. It is important to understand that under no circumstances can a student drive himself or other students to an off-site athletic activity. They must be driven by school provided transportation or parents.
If a new medical condition emerges during the course of the season, please contact the coaches. Also should any contact information or phone numbers change, please advise the school office so that your form can be updated.
Most of the sports played by our school do not run a high risk of injury however injuries may occur and therefore it is important that your child wear appropriate equipment and obey the instructions of coaches and supervisors in order to remain safe.
It is our intention to provide adequate supervision for all road trips. In all cases, at least one female supervisor will accompany female teams and at least one male supervisor will accompany male teams.
