This is the form to register for SWEETHEART ONLY ages 0-3 years as of July 11th, 2026 and have your invoice for payment sent to you. Will you be adding a State/Regional title crown & sash for $45? If YES then please list your top 3 state/regional choices, titles are first come first serve. If you are not adding a title then please put N/A. Crowns & sashes will be ordered when your deposit AND the title fee is paid and take 4-6 weeks to arrive!

CONTESTANT WAVIER: By typing your name (parents name if under the age of 18) you agree all national fees are NON-REFUNDABLE & NON-TRANSFERABLE! Reigning American Miss, LLC can not held responsible or cannot be held liable for injuries, stolen property, or any unforeseen circumstances while on grounds, at the pageant, or any pageant appearances throughout the year. You agree all photos taken by staff be used for promotion and all judges decisions are final. If you are late to the pageant we will not wait and you will have 1 point off per judge for that event, this is for any required event. If any any point you decide to drop out of competition you are required to contact RAM staff, no fees will be refunded or transferred. A $50 withdrawal fee is due if you drop out of the pageant at any point and the crown/sash must be returned within 5-7 business days. If you were to win Sweetheart do not finish your contract, you will be responsible for returning any awards, crowns, or sashes to the national office and can be fined up to $250. You agree to all the rules listed in the handbook and these can be amended at any time by RAM staff. You agree to all the rules listed in the handbook.
