Informed Consent and Acknowledgment
By signing below, I acknowledge that I, as the parent or legal guardian of the youth participating in Nafees Innovations LLC workshops, have read and fully understand the nature and scope of the activities involved. I consent to my child's participation and understand that all reasonable precautions will be taken to ensure the safety and well-being of my child. I acknowledge that I have had the opportunity to ask questions regarding the program and that my child's participation is voluntary. I agree to assume full responsibility for any risks or injuries, foreseen or unforeseen, that may result from participation in the workshops.

Medical Release and Authorization
In the event that my child requires medical attention during their participation in any Nafees Innovations LLC workshop, I hereby authorize the staff or designated representatives to seek emergency medical care. I understand that reasonable efforts will be made to contact me or the emergency contact listed below. I acknowledge that I am responsible for any medical expenses incurred as a result of such treatment. By signing this form, I release Nafees Innovations LLC, its staff, and representatives from any liability related to medical treatment administered during the program.

Media Release
I hereby grant Nafees Innovations LLC permission to capture, use, and distribute photographs, videos, and other media of my child taken during workshops or other activities for educational, promotional, and marketing purposes. I understand that these images may be used in various forms of media, including but not limited to the Nafees Innovations LLC website, social media, newsletters, and promotional materials. I acknowledge that my child's name will not be used without additional consent, and I waive any rights to compensation or ownership of such media.

Refund Policy
At Nafees Innovations LLC, we strive to provide high-quality educational experiences for all participants. If, for any reason, you need to request a refund for your child’s workshop, please email us at info@nafeesino.com or call 215-888-4431 no later than 48 hours prior to the start of the workshop. Refunds requested after this period may not be granted. We appreciate your understanding and cooperation.

Confirmation and Agreement

I confirm that I have read, understood, and agree to the terms outlined in the Informed Consent and Acknowledgment, Medical Release and Authorization, and Media Release sections above. I have also reviewed and understand the Refund Policy. I certify that all information provided is accurate to the best of my knowledge.
