Prom Boutique 2026 ~ Individual Registration

Welcome!

Are you a high school Junior or Senior girl, eligible to attend your prom or miliatry ball, but unable to afford a gown? The Boutique is an exciting and uplifting event where you'll have thousands of dresses to choose from and a Fairy Godmother who will bring joy to your day. We hope you'll leave the Boutique feeling encouraged and know that you are beautiful just as you are. At our boutique, your volunteer Fairy Godmother will help you find the perfect gown completely free of charge.

Once your registration and waiver are completed you will receive a confirmation email with your appointment time. Be sure to check your emails, and spam folders to ensure you are receiving all communications from The Giving Gown Foundation.

Please consider travel time when choosing a time slot. The Boutique is located at Bammel Church of Christ Teen Center, 2700 Cypress Creek Pkwy, Houston, TX 77068 (located towards the back of the property). The undersigned voluntarily agrees to participate in the 2026 Prom Boutique program sponsored by The Giving Gown Foundation on February 14, 2026 .
The undersigned recognizes that Bammel Church of Christ and The Giving Gown Foundation have not undertaken any duty or responsibility for her safety and the undersigned agrees to assume the full responsibility for all risk of illness, bodily injury, death, disability, and property damage as a result of participating in the Gown Giveaway.
The undersigned recognizes that these risks include possible exposure to COVID-19. By my signature, I hereby state that I understand the risks involved in participating in the Gown Giveaway and willingly and voluntarily accept these risks. By my signature, I hereby surrender any right to seek reimbursement from Bammel Church of Christ, The Giving Gown Foundation and its directors, officers, employees, volunteers and other agents for injury or illness sustained and liability incurred during my participation in the activity described above. By my signature, I warrant that I am not relying on any oral representations, statements or inducement apart from the statements made on this form.
