CJSTC Form-76 Eligibility Review Request

This request opens your GovSource eligibility review file for the Florida Equivalency of Training (EOT) certification pathway.

Completion of this form and submission of the applicable eligibility review fee formally opens your CJSTC Form-76 eligibility review file with the GovSource Selection Center.

Submission of payment does not guarantee eligibility or issuance of Form-76. Eligibility is determined after review and verification of the applicant’s documentation.

Assessment Fee Policy
Eligibility review fees ($195 standard review or $45 Florida re-certification review) are non-refundable once the review process has been initiated.

The following questions help GovSource identify the appropriate certification pathway. These questions do not determine final eligibility.

Step 3 — Applicant Certification

GovSource will review eligibility documentation and determine whether you qualify to attend a Florida Equivalency of Training (EOT) academy.

Eligibility review is based on submission of a complete documentation packet. Applicants are responsible for obtaining required verification documents from their employer and training academy unless otherwise directed by GovSource.

Step 5 — Eligibility Review Fee

An eligibility review fee is required to open your CJSTC Form-76 review file with the GovSource Selection Center.

The standard eligibility review fee is $195. Applicants seeking Florida law enforcement re-certification may qualify for a reduced $45 eligibility review fee.

The appropriate fee will be determined based on the certification pathway indicated above.

Submission of payment does not guarantee eligibility or issuance of Form-76. Eligibility is determined after review and verification of the applicant’s documentation.

Once payment is received, GovSource will send instructions and required forms for submitting your eligibility documentation packet.
