CONSENT AND ACKNOWLEDGE
Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on school property or in home resulting from normal dance, music and/or theater activity or any other activity conducted by the students before, during or after class time. The undersigned gives permission to Studio Guru, Inc., DBA Mountain School of Music and Dance, Mountain Performing Arts, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
PHOTO RELEASE
The school is hereby granted the permission to take photographs of students to use in brochures, website, posters and other advertisements. If you do not agree to this please make a note in the comments in your registration.
PAYMENTS
Class tuition is paid by automatic bank account debit or automatic credit card payment at the time of registration. If you chose a monthly payment tuition plans these are charged on the 28th of the month. Your first payment will charge at the time of registration. If a payment is rejected due to insufficient funds there will be an automatic $20.00 NSF fee that will be charged to the account on file.
WITHDRAWALS
You may withdraw from any school year class in the first week of class and receive a refund for unused classes. After the first week, there are no withdrawals or refunds.
You may withdraw from a summer day camp up to 14 days prior to the start date. Overnight camp there are no withdrawals after March 1.
Music lessons are on going and you may withdraw at anytime. We ask for one month 's notice to discontinue lessons.
To withdraw from a class or lesson please email us your intention. You are not withdrawn until receiving confirmation from us. Please follow up with a call should you not receive an email back to confirm receipt of your withdrawal.
