To facilitate the success of St. Joseph Parish ("Parish") initiatives, we seek your support in delivering our message. By uploading your photo, you grant permission for the Parish to use your and/or your children's photo or video testimonial in support of the Parish.
In consideration of participating in Parish activities and in acknowledgement of the goodwill received, the undersigned grants St. Joseph Parish and The Catholic Bishop of Chicago (collectively "Archdiocesan Parties") the right to use, copyright, and publish in any medium, including print media, social media websites, and other websites, for advertising or promotion ("Advertising"), the undersigned's name, biographical information, their relationship with the Parish, as well as any photographs or videos taken of the undersigned (collectively "Personal Identifiers"), irrespective of whether these visual representations are altered in character or form. Additionally, by including their child's name below, the undersigned authorizes the Archdiocesan Parties to use such child's Personal Identifiers for Advertising as specified herein. The undersigned acknowledges that they have no right to inspect or approve the Advertising or Personal Identifiers before publication, and all visual content and Personal Identifiers will remain the property of the Archdiocesan Parties. As part of the consideration under this Agreement, and to the fullest extent of the law, the undersigned agrees to release, discharge, indemnify, defend, and hold the Archdiocesan Parties and all their agents, employees, successors, and assigns harmless from any and all liabilities, claims, and damages, whether in law or equity, arising from this Agreement and/or the Advertising. By uploading your photo below, you consent to the Parish using your photo for social media, website, email newsletter, and other parish promotional purposes.
