Touch-Up Request Form

Each tattoo comes with one complimentary touch-up if needed (fingers and hands not included).

Once you send me your healed photos, I’ll review them and let you know if a touch-up is necessary.

Please only submit photos that are at least 4 weeks healed, taken in good natural lighting, so I can determine if a touch up is needed or not.

Submitting this form does not confirm your appointment. Your appointment will only be confirmed once I receive your deposit and verify the time and date. Please allow up to 3-7 days for a response.
