The Collective Dance Center as well as its insctructors, contractors, volunteers nd staff are not liable for personal injuries or loss of or damage to personal property. I give permission to The Collective Dance Center to provide and/or seek medical attention for my child should medical treatment be required and I will assume all costs of my child?children's medical bill(s). I am hereby waiving and releasing The Collective Dance Cente, its owners, employees, volunteers and contractors from and against any and all claims, costs, liabilities, expenses or judgements including attorney's fees and court costs arising out of participation in the program, or any illness or injury resulting there from and hereby agree to indemnify and hold harmless The Collective dance Center, its owners, employees, volunteers and contractors from any and all claims.

I give The Collective Dance Center permission to take and use photos and/or video materials of my child for advertising and other purposes.

Instructors at times may make physical corrections for the purpose of alignment, porper technique, and placement, and to prevent any prevebtable injury. I give consent for my student to be physically corrected by instructors.
