Who should use this form?

Non-WMCANZ Ministers applying to the National Board of Ministerial Formation to transfer their ministerial credentials to the Wesleyan Methodist Church of Aotearoa New Zealand.

The following documents MUST be submitted with this application:

A completed Police Vetting Request & Consent Form (you can download this form HERE)

Proof of Identification (Two verified forms of ID - One must be photographic),

Birth Certificate or Proof of Residency/Current Visa (Verified Copy)

Proof of Ministerial Status (Copy of Registration/Ordination Certificate or Ministerial License)

Proof of relevant Academic Credentials (eg: Certified Copy of Degree or Official transcript from education provider).
