Student Acceptance Form - LEC808 Professional Practice

Students must complete this form to accept the expectations of professional practice, where the roles and responsibilities agreed upon between you (the student) and Lead Institute of Higher Education (LIHE) are established in the Professional Practice Handbook. This form must be completed prior to commencing professional practice days. I understand that if I choose to continue with being a student on professional experience where I am usually employed, that each case will be negotiated if I am regarded as either a student or paid employee on my scheduled professional practice days. An additional e-form will be sent in this regard, where all parties must then sign (the early childhood service, the student and LIHE) to agree to terms set out in the form. 3. I understand that I need to complete 30 whole days (8 hours including half an hour lunch break) to complete the requirements of LEC808 Professional Practice. I cannot adjust these hours to allow for a shorter day. I understand that I may be required to start an early or late shift to ensure I gain an understanding of different routines and schedules throughout the day. 5. I understand that if I am not able to attend the intended day of my professional practice, I will firstly contact the service (via phone call not text message) in their hours of operation; secondly I will follow up this phone call with a professionally written email to the early childhood service and the Professional Practice Coordinator Sonika Dhiman, to explain my absence. Where appropriate, I will provide evidence to support my absence and make up these days to meet the 30-day requirement of LEC808. 6. I understand that either myself or the early childhood service may contact the Course Director to intervene regarding meeting the requirements of the professional experience.
