Acknowledgment and Assumption of Risk:

I, the undersigned, hereby acknowledge that my participation in the above-referenced event/activity at The Summit Park District (hereinafter referred to as "Park District") is voluntary. I understand that participation in this event/activity involves certain inherent risks, including but not limited to physical injury, property damage, and other potential hazards. I acknowledge that these risks cannot be eliminated even with the utmost care and that I voluntarily assume all such risks associated with my participation.

Waiver and Release of Liability:

In consideration of being permitted to participate in the event/activity, I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, and discharge the Park District, its officers, agents, employees, volunteers, and any other participants, sponsors, or advertisers (hereinafter referred to as "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me, whether caused by the negligence of the Released Parties or otherwise, while participating in the event/activity or while on the premises of the Park District.
