Privacy Statement – Beginning Active Recovery (BAR) Support Group

Your privacy and confidentiality are our top priorities. Participation in BAR is voluntary and confidential. No medical records will be created for this group, and no clinical notes will be kept. Only basic registration information (such as your name and contact details) will be collected to confirm attendance.

Your name, association with BAR, or attendance will never be reported to your agency or employer. De-identified statistics may be shared with the It’s a Calling Foundation for program evaluation and funding purposes, but these reports will never include any personally identifying information.

All personal information is securely maintained by Code-4 Counseling in accordance with our privacy policies.

While we cannot guarantee that other participants will not share your attendance, we STRONGLY expect and request that all members respect and maintain confidentiality regarding one another’s participation and what is shared in the group.
