  • Rural BC Community Award Nomination Form

    This award is given to one BC RSA community to recognize collaboration, partnership, and engagement efforts that strengthen rural healthcare service delivery and community well-being.
  • Community Innovation
    Has the nominated community: 

    • Developed or adapted a community-wide solution to address local healthcare needs?  
    • Created or enhanced initiatives to address social determinant of health, directly improving local patient and community health outcomes or access to care? 

    Inclusive Collaboration
    Has the nominated community:  

    • Applied a collaborative, partnership-based approach to addres local healthcare issue(s)? 
    • Actively engaged with a diverse range of local, regional and/or provincial agencies, organizations, healthcare providers, community members, and patients to co-create solutions?

    Primary and Integrated Care
    Do local rural health care providers and/or administrators: 

    • Focused on establishing, strengthening, or expanding integrated primary healthcare services accessible to all community members?
    • Support team-based, interdisciplinary model of healthcare service delivery that includes allied health, mental health, and social support? 

    *Please attach a list of the different providers and allied support teams working to provide primary care supports in the community. 

    Rural Generalism
    Do local rural health care providers and/or administrators in the nominated community: 

    • Step in to provide secondary and/or tertiary care when necessary, addressing transport barriers and unique holistic rural healthcare considerations
    • Collaborate with specialists across rural, regional and urban areas to provide local delivery of specialist services/telehealth visits allowing for ‘closer-to-home’ care when appropriate?

    In addition to your nomination form, please submit a minimum of two (2) letters of support from different types of partners, such as health administrators, policymakers, academics/researchers, community leaders, and health professionals. These letters should highlight the community’s contributions to partnership, collaboration, and rural healthcare excellence in alignment with the BC Rural Health Awards criteria.
