    Covid-19 Procedures, Health, and Safety: To protect the health of our staff we ask that you ensure you are in accordance with the following guidelines before booking an appointment. If you have an appointment reserved and do not meet these guidelines we ask that you reschedule your appointment to avoid a cancellation fee. Please do not book an appointment if you are not in accordance with the following; I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that STUDIO FIVE TWELVE has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, and all other communicable diseases/infections. I further acknowledge that STUDIO FIVE TWELVE can not guarantee that I will not become infected with the Coronavirus/Covid-19 or any other communicable disease/infection. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 or any other disease/infection may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families. I voluntarily seek services provided by Cindy Johnson and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment. I attest that: * I am not experiencing any symptoms of illness such as cough, shortness of breath, difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. * I have not traveled internationally within the last 14 days. * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.  * I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities. * I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. For your comfort and safety, please notify your stylists if you have allergies, any physical issues or disabilities, or if you are pregnant. If you experience discomfort or any other symptoms during the treatment, please alert your service provider immediately.

    Cancellation Policy: I understand that things can come up. I value your time, and hope you value mine too. In the event you need to cancel your appointment, please be sure to notify me no later than 24 hours before your scheduled appointment. Failure to notify in time will incur a cancellation fee of 50% of scheduled service. This allows other clients to have the option to book an appointment during the time slot that is being cancelled. I understand that emergencies come up and will do my best to work with you. However, if multiple cancellations are made, a non-refundable 50% deposit will be required to book a future appointment. If you have signs of Covid on the same day as your appointment, the cancellation fee will be waived. Thank you for understanding. Service Adjustment Policy: Your satisfaction is the highest priority. If you are not completely content, please let me know during your service or within 7 days after your service has been performed.  I am committed to making any needed corrections, as long as it falls in the services discussed and agreed upon during the consultation. I will be pleased to accommodate you free of charge in most circumstances. For said complimentary service to be done, this request must be made within 7 days of your initial appointment. There are no refunds for services rendered. If there is a change in mind about the color, adjustments will be made and treated as a new service. It may take several sessions to achieve brighter blondes to keep the integrity of the hair in a healthy state. Retail Return Policy: Should you make a retail purchase that you are unsatisfied with, I will except retail exchanges at full credit for 14 days from your initial purchase. Retail refunds are not available. I will exchange your purchased product for another product that I believe is better suited to meet your needs and goals. Child Safety Policy: I welcome children under the age of 16 in the salon only if they are receiving a service themselves. Children may not accompany a parent to any appointment unless they too are booked for a service of the same length at the same time with another stylist in the salon. Salons can be dangerous for children who can fall, cut themselves, or be exposed to potentially harmful chemicals. I pride myself on creating a relaxing experience for all quests, and children can cause a distraction for both the stylists and the other guests in the salon. I love seeing photos of and hearing stories about your children; however, for their safety of all others in the salon, I do ask that you make child care arrangements during your appointments. Salon Etiquette: Discriminatory/Derogatory statements and actions are prohibited in the salon and will not be tolerated. Our staff has the right to refuse service to anyone behaving improperly, intoxicated or if their state of health may influence the effects of the service. Confidentiality and Privacy: Every client has the right to privacy and confidentiality. The conversations you have with your stylist will remain confidential. Client information and email lists are confidential and will not be sold or shared with outside companies.

    ​By reading and agreeing to this statement, I have read and understood all of Cindy Johnson's Policies found in the Digital Welcome Packet and stated in this memo. I also understand that Cindy Johnson and I will have an in depth conversation speaking about all aspects of today's appointment, in which I agree to and acknowledge, including price, and the expected physical and aesthetic results. I hereby release and agree to hold Cindy Johnson harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Cindy Johnson. I understand that this release discharges Cindy Johnson from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Cindy Johnson. This liability waiver and release extends to the salon together with all owners, partners, and employees.

    Hair Extension Aftercare and Maintenance Agreement

    1. Proper Aftercare:

       - I understand that proper aftercare is essential to maintain the quality, longevity, and appearance of my hair extensions.

       - I will follow the provided aftercare instructions, including washing, drying, brushing, and styling techniques, as demonstrated and explained to me by my hair extension professional.

    2. Regular Maintenance:

       - I acknowledge that regular maintenance appointments are necessary to ensure the health and integrity of my natural hair and extensions.

       - I will schedule and attend maintenance appointments at the recommended intervals as advised by my hair extension professional.


    3. Non-Refundable Purchase:

       - I understand and agree that the purchase of hair extensions and related services is non-refundable once the installation has been completed.

       - Any concerns or issues with the extensions or services provided should be addressed within 14 days of the installation.
