This application should take about 30 minutes to complete, but may take less/longer depending on various factors. Additionally, the Up Bank creation will likely take up to 24 hours, so you will likely need to save the form and come back once the account activation is completed.

The first step is to confirm your eligibility. Not everyone is eligible to be part of the platform, and further, not everyone is available for the full payout. Once you've answered the below questions, your eligibility will be calculated and shown at the bottom of the page.

Let's talk the fun stuff. Let's see how much you'll get paid. The final payout amount will vary depending on a range of factors. Be as accurate here as you can be, as typo's or mistakes may hold up the payout process.

Please note, this payout amount is as a guide and not a guranteed payout amount. Once calculated, and the form is completed, our team will need to manually verify some details first which might affect this payout amount.

Any of the below accounts you don't have, we will create as part of your application.

Reminder: At minimum, you must have or be eligible to create over a Bet365 or Sportsbet account. If you can't, your application will be rejected during the review stage.

IMPORTANT – PLEASE READ CAREFULLY

Thank you for completing the first part of the application. Before we can finalise your onboarding and process your ${calculatedPayout} payment, we are required to collect identification documents to meet verification requirements for Australian bookmakers and digital banking providers. This process ensures that all accounts are lawfully created and that we can operate in line with relevant financial and anti-money laundering legislation.

The information you provide below will be used only for the purpose of:

• Verifying your identity with Australian bookmakers (such as TAB, Sportsbet, Pointsbet, etc.);
• Verifying your identity with any other accounts you hand over to us
• Ensuring all accounts are linked appropriately and securely to your identity to avoid fraud, errors, or duplicate registrations.

⚠️ Please note: You remain the legal owner of all accounts created in your name. Cavalry Accounts will operate the accounts on your behalf under a clear and limited-use agreement for the duration of the program. You are not applying for credit, and we will not use your identity for any purposes other than those clearly outlined here.

We take your privacy and data protection seriously. All personal information and identification documents you provide are stored securely using industry-grade encryption and 2-factor authentication systems. Access is strictly limited to authorised personnel at Cavalry Accounts. If at any time you wish to withdraw from the program or request deletion of your data, you may contact us and we will action your request immediately.

This form collects your details to assist in creating and managing betting accounts on regulated Australian platforms. You will remain the legal account holder, and all actions are undertaken with your consent.

Why we require ID

We collect ID details to comply with Australian government legislation. We will also create online accounts using the details you provide with a select number of fully regulated Australian bookmaking websites, e.g. TAB, Sportsbet, Pointsbet etc.

As part of the terms of this agreement, we will never use your ID for any purpose other than to sign up to the aforementioned websites or managing your bank account for the express purpose of depositing into and withdrawing from aforementioned websites.

The following documents are required to complete your application. Please ensure that all uploads are clear, readable, and show all corners of your ID documents.

An important part of the signup process is the creation of a Up Bank account. We get you to create the account and enter the login details into the form below.

Why we need an Up Bank account?
This is for us to deposit into and withdraw funds with the betting providers. As the betting providers require payments to go into an account in your name. Any funds added and removed to this account belong to the members of Cavalry Accounts and is operated by Cavalry Accounts on behalf of you only for the purpose of depositing and withdrawing into sportsbetting accounts.

As to why Up Bank? We aim to standardise our processes as much as possible, and many people don't currently have an Up Bank account. You can learn move about Up Bank at up.com.au.

By signing below, you confirm your agreement to the following:

Cavalry Accounts is authorised to assist in the creation of:

• A digital bank account in your name (e.g. Up Bank), and
• Betting accounts on Australian-licensed bookmakers (e.g. TAB, Sportsbet, Bet365).

These accounts will be linked to a new phone number and email, provided by us, to ensure your personal details are not used for marketing purposes and to maintain operational efficiency.

Your signature authorises us to:

(A) Create and manage betting and banking accounts on your behalf for the purposes outlined;
(B) Communicate with those companies on your behalf to assist with verification and support;
(C) Contact you via SMS or email about program updates or payment — you can opt out at any time.

💰 Reminder: Once your accounts are fully verified and active, you will receive a one-time payment via your nominated bank account.
