• Client/Family Consent Form for Filming and Photography

  • Introduction

    Unity Care Solutions’ staff support children, young and vulnerable adults as part of our service. Photographs and videos can be a source of pleasure and pride and we believe that the use of images can enhance the wellbeing of the person and their families and as such, is something to be welcomed and appreciated. This document covers the use of all film and electronic photographic devices including cameras, mobile phones, tablets and portable gaming devices with inbuilt cameras

    The Purpose

    We would like to use images of our staff and clients for many reasons including:          

    •             Recording activities

    •             Recording learning and development

    •             Celebrating special events and achievements

    These images may be used internally for training purposes; and also, for public relations (including on our website and social media platforms, and printed publications such as promotional leaflets).

    We do however, recognise that with the increased use of technologies, particularly digital and on-line, the potential for misuse has become greater and we understand that this can give cause for concern.  We will endeavour to reassure you by ensuring effective safeguards are in place to protect our staff and clients in minimising risk.

    We have specific policies regarding the use of images and the safe use of mobile and smart devices which will accompany this consent form.

    To comply with UK General Data Protection Regulations (UK GDPR) and the Data Protection Act, we require consent before we can take images of staff and clients.  This form is for gaining our staff member’s consent. 

    Conditions of use:

    1.      This form is valid for the period of time the staff is part of Unity Care Solutions. Images of the staff will not be used after this time and will be destroyed in line with UK GDPR guidelines.

    2.      Under UK GDPR and Data Protection legislation your rights include:

    a)      Consent to the publication of your photo can be withdrawn at any time (principle 1 of the Act)

    b)     The image will not be used for any other purpose without further consent (principle 2 of the Act)

    c)      Personal data will be accurately maintained and kept up to date (principle 4 of the Act)

    d)     Publication of images will cease, and all electronic copies will be deleted when your leave the setting (principle 5 of the Act).

    3.      The images used will be of activities that show our staff and clients in a positive light.  We will only use images of staff and clients who are suitably dressed.  Embarrassing or distressing images will not be used. The images will not be associated with any negative or sensitive issues.

    4.      We may use group photographs or footage with very general labels e.g. training sessions, social events.

    5.      We will make every effort to ensure that we do not allow images to be taken of any staff member for whom we do not have permission to or who are ‘at risk’ or ‘disallowed’ from having their photographs taken for legal or social reasons.

    We will take all reasonable measures to ensure images are used solely for the purposes for which they are intended. However, we cannot guarantee this and take no responsibility for the way images are used by other websites or social media platforms or for any consequences arising from publication.

The person signing on behalf of the client will be checked against the information we have on the client’s support plan under the ‘ Mental Capacity’ section.
