Arts Academy 2025/26 Application Form

Please fill out the form below to apply for the Arts Academy at Crown Heights UMC! The program meets each Thursday that school is in session from 5:30pm to 6:30pm beginning September 18, 2025. Address is 1021 NW 37th St., Oklahoma City, 73118. There is a one-time fee of $10 for the entire year. Pay in person or pay through the online portal on the church homepage.
