This application is available until November 1st. We serve children 0-16 and special needs clients who live in Dallas, High Shoals, Stanley and Alexis with at least one want, one need and one education item. You will need to provide income information, documentation of residency, proof of guardianship, etc.

Please note that you will need to upload documents to this application before submitting. Please have all documentation ready before starting the application process. We will work with you on documents however, you MUST submit Proof of address such as a utility bill, lease, letter from landlord, etc, Photo ID of guardian and Proof that the child exist such as Birth Certificate, School Paperwork, Doctors forms (it must have the child's name and date of birth listed.) You can submit the application without all documents, however, your application will not be approved until these items are submitted.
