I/We wish to participate in baseball/softball-related activities at 008 Training, located at 1630 Challenge Drive, Unit 110, Concord, CA 94520. I understand that participation in training, lessons, cage rentals, camps, and related activities involves inherent risks, including but not limited to: impacts from balls or equipment, collisions with other participants, slips, trips, falls, muscle strains, sprains, fractures, or other accidents. I acknowledge and fully accept these risks and voluntarily assume full responsibility for participation.
In consideration of being allowed to participate in activities at 008 Training, I hereby release and discharge 008 Training, along with its owners, coaches, directors, employees, contractors, and representatives, from any and all claims or demands for injury, illness, property damage, or other loss that may arise from participation in training, lessons, rentals, camps, or related activities. I understand and agree that this release applies regardless of the cause, including the ordinary negligence of the parties listed above, to the fullest extent permitted by law.
In the event of an injury or medical emergency, I authorize 008 Training staff to secure necessary medical treatment for myself or my child. I understand that I am responsible for all associated costs of such treatment.
By signing below, I acknowledge that I have read, understand, and voluntarily agree to the terms of this Release of Liability and Assumption of Risk Agreement. I confirm that I am the parent/legal guardian of the participant listed above (or the participant if 18+).
