  • Our goal is to simplify the grant application process by including only relevant questions that aid us in making a decision. Please note that we prioritize the quality of your project and its impact on the community over writing style or grammar. We set a suggested word count for each question to encourage you to provide only the necessary details. While you will not be able to exceed the suggested maximum word count, do not worry about reaching it. Just tell us what you want us to know.

  • Please submit your requested amount in the field below and follow these guidelines:

    • A single municipality is eligible for a matching grant of up to $50,000. Municipalities collaborating with a nonprofit organization can have the nonprofit be the lead applicant if that leads to greater efficiency.
    • Municipalities can collaborate by pooling their resources and funds to pursue a regional grant of up to $150,000. Mosaic may fund beyond a one-to-one match for regional collaboratives only. This approach is especially beneficial for communities struggling to support a project independently due to limited abatement funds. By joining forces as a collaborative group, these municipalities can request more funding than they can individually contribute. Funding requests from these regional collaboratives will be carefully evaluated, focusing on how well they align with the goals of the proposed initiative. This strategy encourages cooperation and supports communities working together to have a greater impact.

  • Please provide proof of the municipality's (or municipalities') commitment to allocate opioid abatement funds for the proposal. Such proof may be 1) a formal recorded budget discussion or 2) a commitment letter signed by the mayor or city/town manager. The letter should be on municipal letterhead with the municipality's address and must contain (at minimum) the following text:

     

    To Whom It May Concern,

    The City/Town of XYZ confirms its commitment to allocate $0.00 of its opioid abatement funds to the Mosaic Municipal Matching Grant program. 

     

    HOWEVER: IF the applicant is a regional collaborative OR a nonprofit partnering with a collaborative AND the proof is not available when the application is submitted, a letter certifying the intent of the collaborative to commit matching funds is sufficient to submit with the application. If a letter of intent is submitted with the application, proof of the above commitment must be submitted within 5 days of receiving notice of a grant award.
