I, (print name), hereby authorize The Mission Cancer Foundation(“Foundation”) to use my name, likeness, image, voice, and/or written or spoken statements forpromotional, educational, and marketing purposes. This authorization includes, but is not limitedto, photographs, video and audio recordings, electronic content, digital images, and testimonialscreated by or provided to the Foundation.
I understand that these materials may be used to communicate the services and mission of theFoundation in formats such as (but not limited to): brochures, posters, websites, social media,videos, interviews, press/media coverage, and other electronic or print communications. All suchmaterials shall remain the sole property of the Foundation.
I grant permission for the Foundation, as well as its authorized agents, contractors, and partners,to create, publish, distribute, and disseminate these materials. I understand that I will receive nofinancial compensation in connection with their use and that I waive any right to review, approve, oredit the final content prior to publication.
I understand there will be no compensation associated with the use of the above-referencedmaterials. I waive the right to inspect or approve the final content created. I agree to release andhold harmless the Foundation and its agents, Board members, employees, interns and volunteers,and any firm it may select to publish and distribute my image(s) in whole or in part.
Unless otherwise noted, this authorization is granted in perpetuity. Any restrictions, expirationdates, or conditions regarding the use of my materials or identifying information are as follows(optional):
