As the parent or legal guardian of the child named above, I hereby give my full consent and approval for my child to participate in the STREET JAM KIDZ program presented by Bama Against Bullying and/or Barbie Perry being held at the YMCA Benjamin Barnes in Tuscaloosa, Alabama. I hereby certify that my child is fully capable of participating and that my child is in healthy condition. In addition to giving my full consent for my child’s participation, I do hereby waive, release and hold harmless Bama Against Bullying and/or Barbie Perry its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the designated program. I hereby assign full copyright of any photographs or video taken of my child together with the right of reproduction either wholly or in part. I understand that the videos created in class will be shared on social media. Bama Against Bullying and/or Barbie Perry, assume no responsibility or liability for lost, stolen or misplaced items. By signing, I agree to hold the above mentioned entirely free from any liability, including financial responsibility for injuries occurred, regardless of whether injuries occurred by negligence. I also forfeit all rights to bring a suit against them for any reason.
