Girly Girl I Care is committed to supporting young girls by providing essential hygiene products and appropriate clothing items. Our goal is to promote health, confidence, and dignity among youth in our community.

To ensure we have permission to provide these items we kindly ask that you complete and return this signed consent form All items are provided free of charge and participation is completely voluntary

Items That May Be Distributed Include (but are not limited to): Feminine hygiene products (pads, liners, tampons) Body wash, deodorant, lotion, toothpaste, toothbrushes Undergarments, socks, and weather-appropriate clothing

Consent Statement

I, the undersigned, am the parent or legal guardian of the child named above. I hereby give permission for my child to receive hygiene products and clothing from Girly Girl I Care.

I understand that these items are provided as part of a charitable outreach program, and that participation does not require payment. I acknowledge that I may contact the organization at any time with questions or to revoke this consent.
