Winter sports sign-ups close on October 12th, 2026. All required paperwork must be submitted by October 19th, 2026. Students who do not turn in their paperwork by the October 19th deadline may not be eligible to practice or play until all required paperwork has been submitted.

Athletic Forms

Athletes must complete the athletic forms below, as required by Summit Christian Academy and TAPPS. Completed forms can be scanned to athletics@summiteagles.org or hard copies can be turned in to the front office.
- Consent and Liability Release (all athletes)
- Athletics Code of Code (all athletes)
- TAPPS Medical History Form (all athletes)
- TAPPS Physical Form (middle school and high school athletes only)
