Team Registration

Fill out this form to register for the event or turn in a printed form to the SCS front office. Registration Deadline: October 10
This tournament is open to all students in grades K-8, high school and community members 18+ (parents, caregivers, staff, alumni, etc.)

Team Divisions will be separated as follows: K-2, 3-5, 6-8, High School, Adult

Event Day: Saturday October 18 (rain date October 19)

Games will start at 10am, however once registration is closed and brackets have been finalized, teams will be informed of the final schedule including exact division check in times and starting game time

*All teams must be made of 2 players only, no singles allowed.

TEAM ENTRY FEE: Adult division $40 per team / Student Division $30 per team (you must submit payment for each team member included on this form no later than Friday October 10th to be confirmed as registered)

How to submit payment:

1. Bring a cash or check made out to SCS PTO to the SCS front office in a sealed envelope with your Team Name and Player Names.

2. Pay via venmo or paypal (YOU MUST WRITE "CORNHOLE" AND "YOUR TEAM NAME" IN NOTES SECTION WHEN SUBMITTING PAYMENT)

There will be prizes for winning teams in each division.

**All proceeds from the event will go directly to the SCS PTO to support school initiatives and classroom funding. To learn more about what where the money goes and initiatives we have supported in the past go to shelburnepto.org
