BC Family Hearing Resource Society (BCFHRS) provides both in-person and online services. To optimize service delivery, it can be useful for you to receive some services online. This may be done using video streaming applications. We need your permission to provide our services via these online options.

Video streaming applications allow users to transmit video over the internet, and/or to share various kinds of files. You need to be aware that sending information via these public video streaming applications e.g. Zoom/FaceTime/WhatsApp/Microsoft Teams, includes a risk of personal information being accidentally disclosed to other people (e.g. on the web).
Do you authorize BCFHRS to provide services to your family online?*

Yes, I give permission to BCFHRS to provide services through the platforms indicated below.No, I do not give permission.
Zoom, FaceTime, WhatsApp and Microsoft Teams are more secure compared to other streaming applications and, as such, are preferable for the services we provide. Please select all the options that you consent to use:
