    Please complete this form in its entirety to submit an application for the 2016 Measure B Bicycle and Pedestrian Planning Studies Competitive Grant Application for Cycle 3. 

    • Before filling out this application form, ensure the proposed project meets the Program Screening Criteriam as noted in the 2016 Measure B Bicycle and Pedestrian Planning Studies Competitive Grant Program Criteria. If the proposed project does not meet the Program Screening Criteria, the application will be rejected. 
    • Utilize the 2016 Measure B Bicycle and Pedestrian Planning Studies Competitive Grant Application Guidelines for additional guidance.
    • Include additional information as an attachment if more space is required to answer the questions as indicated at the end of the form. Please make clear references to any attachments in your narratives. 
    • Submit one application for each project. Incomplete applications will not be accepted

    All signed applications are due by 4:00PM, Monday, February 9, 2026.

    Submit Your Application

    Once you submit your application, please keep a copy of the confirmation email. Using the link in the email, you may access your application and make edits until the deadline.

    All changes must be saved and submitted before the 4:00 PM deadline.

    Signauture Verification

    If you are logged into a Jotform account: The signee must be logged into the same account that is associated with the application. If the signee is not the one filling out the form, you must share the login information with the signee. 

    If you have skipped creating an account: You may share the shareable link with the signee when ready. This is the recommended method so that you do not need to share login information. 

    Attachments

    The maximum file size for each attachment is 10 MB. You can upload multiple files for each prompt. Additional attachments may be afixed to the end of the application if needed.

    IMPORTANT: The application must be signed by Public Works Director, Agency Manager, or equivalent senior-level staff or their designee.
  • Signature Instruction

    • Applications must be signed to be considered complete and submitted.

    • The signee should review all answers before signing. 

    • Signatures are certified through DocuSign, where the signee will be redirected to. The signee does not need a DocuSign account.
