Liability Release
I do hereby release, forever discharge, and agree to hold harmless the Churches in Athens, Atlanta, Kennesaw, Birmingham, Huntsville, Knoxville, Nashville, Memphis, Murfreesboro, Charlotte, Raleigh, Chapel Hill, Columbia, Simpsonville, Chattanooga and the staff and volunteers thereof from any liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the participant that occur while said participant is participating in the above described activity.

Furthermore, I assume all risk or personal injury, sickness, death, damage, and expense as a result of participation in transportation, recreation, and work activities involved therein.

The undersigned further hereby agrees to hold harmless and indemnify said church, its directors, employees, and agents, for any liability sustained by said church as the result of the negligent, willful, or intentional acts of said participant, including expenses incurred attendant thereto.

I understand that from time to time activities during the Southeast Christian Students Conference may be photographed or recorded. By signing this form, I give permission to the Churches in Athens, Atlanta, Kennesaw, Birmingham, Huntsville, Knoxville, Nashville, Memphis, Murfreesboro, Charlotte, Raleigh, Chapel Hill, Columbia, Simpsonville, Chattanooga to use any photographs and/or videos of me taken while participating in activities of the Southeast Christian Students Conference in the digital and print communications (“publications”) of the churches and respective Student Organizations. The aforementioned churches and student organizations will never publish your name or any other personal information with any of its publications without your written permission.
