Attendance policy
For Participants Age 18 and Over
By signing this form, I confirm that I am at least 18 years old and legally responsible for my own participation. I acknowledge that folklor dance involves physical activity and that there are inherent risks of injury. I agree to participate voluntarily, accept responsibility for my own health and safety, and release Opancici Serbian Cultural Center, its instructors, and organizers from liability for accidental injuries.

By signing this form, I confirm that I am the parent or legal guardian of the participant(s) named above. I grant permission for my child(ren) to participate in their respective folklor dance group. I understand the nature of the activities involved and acknowledge that there are inherent risks of injury. I release Opancici Serbian Cultural Center, its instructors, and organizers from any liability for accidental injuries. I also authorize program staff to obtain emergency medical treatment for my child if necessary, with the understanding that I will be responsible for any associated medical expenses.
