I consent to the photography and/or filming of myself and give permission for Nottingham Playhouse and / or its partners to use this photography and/or film for promotional purposes. This includes TV broadcasting, publication press releases, print, online on social media channels and the website, and in emails to the Nottingham Playhouse mailing list. Photographs and/or filming will sometimes be shared amongst the group and parents/guardians.

I understand that Nottingham Playhouse has a Safeguarding Children & Vulnerable Adults Policy, which includes taking every precaution to ensure that images/videos are stored securely, and those who do not consent to be filmed/photographed cannot be identified even in the background of published images/videos.

I also give my permission for this to be stored by Nottingham Playhouse in either a digital or hard copy for an indefinite period. I understand that if I change my mind about the storage, I may contact Nottingham Playhouse and the footage will be destroyed.
