Consent & Acknowledgment

1. I am at least 18 years old or 16+ with a parent/guardian present.

2. I have read and understand the artist’s policies.

3. I am not under the influence of alcohol or drugs.

4. I understand there are risks of infection, allergic reaction, or scarring, and proper aftercare is my responsibility.

5. I release Inked by Kaela and all associated parties from any liability related to my tattoo.

6. All artwork is original or approved for tattooing.
