YOUTH ACTIVITY WAIVER AND RELEASE OF LIABILITY
I voluntarily participate, or allow my child to participate, in Gaines Avenue Church of Christ Youth Activities. I understand that participation may involve risks of injury and that not all risks can be anticipated.
I, for myself and on behalf of my child(ren), voluntarily release and hold harmless Gaines Avenue Church of Christ, its officers, ministers, employees, volunteers, and representatives from claims, liabilities, damages, or expenses arising from participation in the Activity, to the fullest extent permitted by law.
I authorize Gaines Avenue Church of Christ or its representatives to obtain emergency medical care for me or my child if necessary, including first aid, CPR, and emergency transportation. I understand that I am responsible for all medical expenses and for maintaining appropriate health insurance coverage.
I also agree to be responsible for damage to church or Activity property caused by my neglect, misuse, or reckless behavior, or that of my child.
By signing below, I confirm that I have read, understand, and voluntarily agree to this Waiver and Release of Liability.
