  • I understand and am aware that I will be getting Gel-X (Soft gel extension system) applied to each of my natural nails. Gel-X is applied to the natural nail using nail gel and then cured under a UV LED light. I agree to have Gel-X extensions be applied to my nails or removed. By signing this agreement, I consent this service by Diana N. to be performed.


    I have informed my nail technician all my medical conditions. (Current allergies/sensitivities, current use of any nail products & etc.)


    This agreement will remain in effect for this service and future services & I understand this agreement is legal & binding. I have full read each statement and understand all information.

  • Have you had Gel-X extensions before ?
  • If you are experiencing fungus on nails or Covid/Flu/Cold symptoms or test positive for any before or on the day of your appointment, you will be responsible for canceling or rescheduling your appointment.
  • I understand and agree to follow aftercare steps. I understand that incorrect or lack of aftercare may result in damage to natural nails or infection & I agree to take on that risk.
  • I understand if I have damaged nails or any sort of infection I am responsible of canceling my appointment
  • I herby consent and authorize Diana Navas to take pictures and allow them to post me on social media platforms. (Instgram, Tiktok or Facebook.. Please note that I will always ask if you are comfortable in me taking pictures of you)
  • I herby consent and authorize the person written below to perform Gel X extension on my nails.
