MPTF Application Form
This application is to request sponsorship funding from the Music Performance Trust Fund. MPTF sponsorship is used to support free public concerts. All HMG members are welcome to apply. Payment is at scale. The HMG and MPTF are not responsible for any changes, errors or omissions after the application has been submitted. All fields must be completed; if you have no answer, type in N/A. Please submit a separate application for each MPTF gig. MPTF applications cannot be considered unless all required information is provided. A submission does not guarantee funding. If you have any questions, contact the HMG at local293hmg@bellnet.ca.

Please give a general description of the gig. Will it be free and open to all members of the public, or will there be an admission fee or similar requirement?
What fee have you negotiated with the event host for the gig? Always negotiate for the highest amount you can get; typically the MPTF funds subsidize the event host at 50% of Scale so they can pay you more. The exceptions to this involve seniors' residences and schools where the MPTF pays 100% of Scale.

If approved, all application details will be shared with all band members. One cheque will be provided to the Band Leader (less 5% work dues), to be distributed among band members. All band members will be informed by the HMG office as to the net pay each member should expect to receive from this gig. Once the gig is done, please do not call the office looking for your cheque. We have to confirm the gig with the MPTF office in New York, they prepare their cheque and send it to us. Then we can pay you. It typically takes a couple of weeks. We don’t have the MPTF funds sitting in a bank account to pay you right away.
