MPTF Application Form
This application is to request sponsorship funding from the Music Performance Trust Fund. MPTF sponsorship is used to support free public concerts. All HMG members are welcome to apply. Payment is at scale. The HMG and MPTF are not responsible for any changes, errors or omissions after the application has been submitted. All fields must be completed; if you have no answer, type in N/A. Please submit a separate application for each MPTF gig. MPTF applications cannot be considered unless all required information is provided. A submission does not guarantee funding. If you have any questions, contact the HMG at local293hmg@bellnet.ca.

If approved, all application details will be shared with all band members. One cheque will be provided to the Band Leader (less 5% work dues), to be distributed among band members. All band members will be informed by the HMG office as to the net pay each member should expect to receive from this gig.
