Electronic Communication Consent Form

This form allows homeowners and renters to opt in to receive HOA communications by email and/or text (SMS), including community updates, emergency notices, and, for homeowners only, election-related materials.
Please complete the form below and provide your contact preferences and authorization.

You may choose any or all depending on your preference:

Email Notices – meeting announcements, community updates, election information, and other Association notices.
Text (SMS) Messages – time-sensitive alerts, maintenance or emergency updates, and reminders.
Electronic Voting Access – to participate in online elections or ballots conducted through the Association’s designated electronic voting system or vendor (including, but not limited to, Pilera or BallotBliss).

PLEASE NOTE: Only homeowners may participate in elections or voting. Tenants are not elgilble.

By completing this form, I/We consent to receive community updates, emergency alerts, and official communications from the Copper Creek of Riverview Homeowners Association, Inc. (“Association”) via email and/or text (SMS), using any platform the Association may adopt now or in the future. This consent remains valid unless revoked in writing, and I/We understand it is our responsibility to keep contact information current.

Voting and Elections:
Only deeded homeowners (or designated voting representatives) are eligible to receive election-related notices and participate in electronic voting. Renters will not receive voting materials.

TEXT

I consent to receive HOA general and emergency alerts by text (SMS) for Time-sensitive alerts, maintenance or emergency updates, and reminders.

ELECTRONIC VOTING

I consent (homeowners only) to receive election and voting notices electronically through HOA-approved voting platforms and to participate in online elections or ballots conducted through the Association’s designated electronic voting system or vendor and to receive notifications from such systems.

Voting access is available to homeowners only.

Consent and Authorization

By signing below, I/We consent to receive official notices and communications from Copper Creek of Riverview Homeowners Association, Inc. by electronic transmission (email and/or text message) as permitted by Florida Statute §720.303(2).

This consent remains valid until revoked in writing by the homeowner or tenant. It is my/our responsibility to keep contact information current and to notify the Association of any changes.

I/We understand that this consent may be revoked at any time by providing written notice to the Association. Upon revocation, I/We will receive required notices by mail or as otherwise permitted by law.

Email addresses and phone numbers provided will be used solely for Association business and will not be shared or sold, in accordance with Florida Statute §720.303(4).
