Read before signing, as this constitutes the agreement as a volunteer and the understanding of your working relationship with Lutheran Disaster Response Carolinas.
In consideration of participating with LDR Carolinas, North Carolina Synod, South Carolina Synod, Evangelical Lutheran Church in America and Lutheran Disaster Response (LDRC, NCS, SCS, ELCA, & LDR) in participation in disaster recovery work, in the Western North Carolina region of North Carolina, during the dates listed above as “Volunteer Dates” I, the undersigned, fully recognize the dangers and hazards inherent in disaster recovery work, and any transportation, including personal injury, property damage, or wrongful death, as well as the unknown dangers and hazards which may arise in the course of my participation, do hereby voluntarily:
Agree, for myself, my heirs and my personal representative, to defend, hold harmless, indemnify, release and forever discharge, to the broadest extent allowed by law, LDRC, NCS, SCS, ELCA, & LDR, their trustees, officers, employees, agents, insurers, successors, assigns from and against any and all claims, demands, actions, or causes of action on account of any damage to real or personal property or any personal injury or death that may result from my participation in the above disaster recovery efforts.
I have read this release, I understand it fully, I understand that it is legally binding, and I understand that, among other things, I am agreeing to indemnify LDRC, NCS, SCS, ELCA, & LDR, for injuries, damages or losses I may cause and giving up rights to sue LDRC, NCS, SCS, ELCA, & LDR for injuries, damages or losses I may incur.
I also release the participant’s name as part of an information database for the LDRC, NCS, SCS, ELCA, & LDR related entities and grant the LDRC, NCS, SCS, ELCA, & LDR unrestricted rights to use, alter, and reproduce any images (still and video) from the event, in any medium without compensation.
